Transition Project Part I: Assessment Plan

PURPOSE: The purpose of this assignment is to gain experience transition planning for a student. Note: these assignments are not part of an actual IEP transition plan. We will complete one of those forms together.

These assignments are more about moving you through a thinking process as you plan for students.

RELEVANCE: All educators are typically involved in the IEP planning process at some point—even general education teachers and related service providers. And most educators will have students with disabilities included in their classrooms. Special educators work as a team with families, students, and a wide range of school-based professionals (e.g., administrators, general educators, related services providers, school counselors) to plan and implement IEPs and to develop transition plans.
PART 1: Assessment/Student characteristics
This section focuses on (a) identifying assessment tools that align with specific transition domains, evaluating assessment tools and their strengths and weaknesses, and modifying assessment tools to fit the needs of a wide range of students.
Using the Transition TN website www.transitiontn.org explore transition assessment tools. You will need to create a login. Make sure to explore the following tool:

Complete the Casey Life Skills assessment as if you were Angelica’s teacher (it’s OK to make things up if needed). Then look at the resources on the website and the report that gets generated to respond to the rubric items.
